WHERE: Irwin County Conference Center on 6th Street
(Old Band Building – located immediately to the right of the Gym)
110 Slots available. Children are accepted on a first-come basis.
Child must be 4 years old on or before September 1, 2012.
Documents Needed:
Child’s Certified Birth Certificate
Immunization Form # 3231
Child’s Social Security Card
Medicaid/PEACH State/Well Care Insurance Card
Eye, Ear, Dental Form #3300
Proof of Irwin County Residency
Proof of Guardianship if not the Biological Parents
Documents used to prove residency must include the name of the parent, guardian, or other person enrolling the student (or the name of the emancipated minor) and a street address.
- Utility Bill (Electric, Water, Telephone or Gas) NOTE: Land Line Telephone records are acceptable; Cell phone bills are not acceptable (Must be within last 2 months)
- U.S. Post Office change of address form (Good for 30 days only)
- Current lease/rental agreement
- Current paycheck stub
- Most recent income tax return
- Current residential property tax statement or bill
- Current home purchase agreement
- Current homeowner’s insurance policy
- Voter registration record or government-issued identification record
- Third-person affidavit of residency (notarized affidavit shall be completed and signed by parent, guardian or other person, as well as the legal owner or leaser of the property where the student and the parent, guardian, or other person resides).
NOTE: An on site visit by a school administrator, resource officer, visiting teacher, school social worker, etc., may be made at a time when residents would be expected to be at home.
Head Start parents must get any documents from Head Start.
